RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Chair of the Board to execute, the attached professional services agreement with Crestwood Behavioral Health, Inc. regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses for the period of July 1, 2022 through June 30, 2025;

2.                     Authorize the Department of Health and Human Services - Behavioral Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached professional services agreement with Crestwood Behavioral Health, Inc. after review and approval by County Counsel, Risk Management and the County Administrative Office; and

3.                     Direct the Clerk of the Board to return one (1) fully executed original copy of the attached professional services agreement with Crestwood Behavioral Health, Inc. to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Humboldt County Department of Health and Human Services - Behavioral Health (“DHHS - Behavioral Health”), in conjunction with the Humboldt County Public Guardian’s Office, utilizes a variety of placement options to provide long-term mental health treatment services to mentally ill patients who require assistance in a secure setting.  It is important for the placement team to have access to a variety of treatment options because vacancies and client needs fluctuate.

 

Crestwood Behavioral Health, Inc. (“Crestwood”) operates a variety of facilities throughout California designed to serve diverse client needs, including, without limitation, an adult residential treatment facility and a secure mental health rehabilitation center, both of which are located in the City of Eureka.

 

Crestwood’s adult residential treatment facility, known as the Pathways program, serves as a sub-acute psychiatric residential facility, offering rehabilitative mental health services for severely and persistently mentally ill persons.  All mental health treatment services provided by Crestwood’s Pathways program focus on transitioning clients to independent living within their communities.

 

Crestwood’s mental health rehabilitation center (“MHRC”) serves clients who require twenty-four (24) hour monitoring in a locked and secure environment due to the severity of their mental illness.  DHHS - Behavioral Health clients placed at Crestwood’s MHRC are monitored by the county’s multi-disciplinary Strategic Assistance for Adult Recovery Interventions (“SAFARI”) committee.  The SAFARI committee authorizes services, determines the appropriate level of care, and monitors clients’ needs in the community upon discharge.  When an initial placement at Crestwood’s MHRC occurs, transitional plans for a less restrictive placement are put into place, and reviewed monthly, in order to transition the client to community living as soon as may be appropriate.  All mental health treatment services provided at Crestwood’s MHRC are delivered with a wellness focus which includes, without limitation, concepts of recovery and resilience.

 

The attached professional services agreement will allow eligible DHHS - Behavioral Health clients to receive mental health treatment services at Crestwood’s adult residential treatment facility and mental health rehabilitation center for the period of July 1, 2022 through June 30, 2025.  It should be noted that the attached professional services agreement with Crestwood comes to the Board after the start date thereof due to inadvertent delays that occurred during the contract development process.

 

Accordingly, DHHS - Behavioral Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached professional services agreement with Crestwood regarding the provision of long-term mental health treatment services to adults with severe mental illnesses for the period of July 1, 2022 through June 30, 2025.  It is also recommended that the Board authorize the DHHS - Behavioral Health Director, or a designee thereof, to execute any future amendments to the attached professional services agreement with Crestwood, after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid any unnecessary delays in the provision of needed mental health treatment services.

 

FINANCIAL IMPACT:

The attached professional services agreement with Crestwood has a maximum amount payable of Seven Million Five Hundred Thousand Dollars ($7,500,000.00) for the period of July 1, 2022 through June 30, 2025.  In no event shall the maximum amount paid under the attached professional services agreement with Crestwood exceed Two Million Five Hundred Thousand Dollars ($2,500,000.00) per fiscal year for fiscal years 2022-2023 through 2024-2025.  However, if local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the attached professional services agreement with Crestwood in its entirety.

 

Expenditure appropriations related to the attached professional services agreement with Crestwood have been included in the approved fiscal year 2022-2023 budget for DHHS - Behavioral Health Adult Services budget unit 1170-496.  Funding for the attached professional services agreement with Crestwood is available through state realignment and Mental Health Services Act revenues received by DHHS - Behavioral Health.  As a result, approval of the attached professional services agreement with Crestwood will not impact the Humboldt County General Fund.

 

STRATEGIC FRAMEWORK:

The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations, supporting self-reliance of citizens, creating opportunities for improved safety and health and providing community-appropriate levels of service.

ALTERNATIVES TO STAFF RECOMMENDATIONS:                     

The Board could choose not to approve the attached professional services agreement with Crestwood regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses.  However, this alternative is not recommended since it would require DHHS - Behavioral Health to place clients at inpatient facilities which will be more costly and may not provide the appropriate level of treatment for such clients.
