First Amendment to the Agreement with Crestwood Behavioral Health, Inc. Placement Facilities for Fiscal Year 2014-15 and 2015-16.
Recommendation
That the Board of Supervisors approve the First Amendment to the Agreement between County of Humboldt and Crestwood Behavioral Health, Inc. (CBHI) Placement Facilities for fiscal year 2014-15 and 2015-16; authorize the Chair to execute three (3) originals of the Amendment; and direct the Clerk of the Board to return two (2) executed originals of the First Amendment to the Department of Health and Human Services (DHHS)-Contract Unit for forwarding to DHHS-Mental Health.
