Agreement between Humboldt County and Crestwood Behavioral Health Inc. Placement Facilities for Fiscal Years 2016-17 through 2018-19
Recommendation
That the Board of Supervisors approve the agreement between Humboldt County and Crestwood Behavioral Health Inc. Placement Facilities for fiscal years 2016-17 through 2018-19; authorize the Chair to execute three (3) originals of the agreement effective July 1,2016; and direct the Clerk of the Board to return two (2) executed originals of the agreement to the Department of Health and Human Services (DHHS)-Contract Unit for forwarding to DHHS-Mental Health Administration.
