RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve and authorize the Director of Department of Health and Human Services (DHHS) - Public Health to execute three original signature pages of State Standard Agreement number 22-10165 and the associated Contractor Certification Clause and California Civil Rights Laws Attachment for the period of July 1, 2022 through June 30, 2027;
2. Authorize the Director of DHHS - Public Health, or their designee thereof, to sign all subsequent amendments and documents directly related to this agreement with County Counsel and Risk Management review and approval.

DISCUSSION:
Humboldt County DHHS - Public Health has been granted an award of $917,090 through a State Standard Agreement from the California Department of Public Health (CDPH) for the period of July 1, 2022 through June 30, 2027 to expand upon the existing Local Oral Health Program (LOHP). In 2017, your Board approved the first round of CDPH grant funds for a LOHP for the period of 2017 through 2022. During the last 5 years, DHHS - Public Health's LOHP has successfully developed a robust program that focuses on building a sustainable infrastructure to support the facilitation and implementation of oral health education, prevention, linkages to treatment, surveillance, and case management services for children and adults in Humboldt County. In 2018 the LOHP partnered with the California Center for Rural Policy (CCRP) to conduct an oral health needs assessment and during 2019 through 2022 completed a Community Health Improvement Plan tailored to the community's needs.
