RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Ratify the Director of Public Works to sign Contract Change Order Number 3 for the total amount of $12,201.96;
2. Accept the completed contract, including all change orders approved by the Public Works Department, between Van Meter Logging, Inc., and Humboldt County for construction of Storm Damage Repair to Blue Slide Road Post Mile 3.24; and
3. Approve the final claim for payment in the amount of $12,201.96, which will bring the total amount paid to 100 percent of the contract.

DISCUSSION:
On Mar. 7, 2017, the Governor of the State of California declared a disaster for Humboldt County due to severe winter storms, flooding, and mudslides. Blue Slide Road Post Mile 3.24, located between Ferndale and Rio Dell in central Humboldt County, experienced damage due to a slide below the roadway, which closed the travel lane for westbound traffic.

On Oct. 29, 2019, the Humboldt County Department of Public Works (Public Works) received funding approval from the Federal Highway Administration to proceed with the Blue Slide Road Post Mile 3.24 Storm Damage Repair Project. Public Works prepared plans and specifications based on a topographic survey of the damaged area prepared by Points West Surveying Company. The work consisted of constructing a welded wire wall with drainage features, installing guardrails, reconstructing the roadway with aggregate base, placing hot mix asphalt, and installing sediment and erosion control measures.
