California Integrated Vital Records System Local Health Department Participant Agreement with California Department of Public Health
Recommendation
That the Board of Supervisors approve the California Integrated Vital Records System (Cal-IVRS) Local Health Department Participant Agreement with California Department of Public Health (CDPH) for a term of five (5) years from the date of final execution by both parties; authorize the County of Humboldt Health Officer or designee to sign the Cal-IVRS Local Health Department Participant Agreement with CDPH; authorize the County of Humboldt Health Officer or designee to sign all future amendments to this Cal-IVRS agreement upon County Counsel and Risk Management review and approval; and direct the Clerk of the Board to return one (1) fully executed copy of the Board order to the DHHS-Contract Unit.
