RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve the California Integrated Vital Records System (Cal-IVRS) Local Health Department Participant Agreement with California Department of Public Health (CDPH) for a term of five (5) years from the date of final execution by both parties;
2. Authorize the County of Humboldt Health Officer or designee to sign the Cal-IVRS Local Health Department Participant Agreement with CDPH;
3. Authorize the County of Humboldt Health Officer or designee to sign all future amendments to this Cal-IVRS agreement upon County Counsel and Risk Management review and approval; and
4. Direct the Clerk of the Board to return one (1) fully executed copy of the Board Order to the DHHS Contract Unit.

DISCUSSION:
CDPH and its Director, designated in statute as the State Registrar, pursuant to Health and Safety Code Sections 102175 through 102249, is charged with the duties of registering, maintaining, indexing, and issuing certified copies of all California birth, death, fetal death, and marriage records.

DHHS - Public Health's Vital Statistics (PHVS) program acts as the administrator for registering and certifying out of hospital and local hospital births. PHVS produces birth certificates for children ages 0-2. PHVS also certifies and registers deaths occurring within the county.

CDPH works with the California Integrated Vital Record System (Cal-IVRS), to review birth, fetal death and death registrations entered by hospital birth clerks, mortuary staff, and deputy local registrars prior to submitting for CDPH Vital Record registration of said record.
