Agreement with California Department of Public Health (CDPH) for the Immunization Program for Fiscal Years 2017-2022
Recommendation
That the Board of Supervisors approve and authorize the Chair of the Board to sign three (3) original signature pages of State Standard Agreement #17-10318, the Civil Rights Laws Attachment and Exhibit F Certification Regarding Lobbying; authorize the Director of Department of Health and Human Services (DHHS) - Public Health or designee to sign any future amendments or budget revisions directly related to this agreement upon review and approval of County Counsel and Risk Management; and direct the Clerk of the Board to return the signed documents and one (1) copy of the Board Motion to the Department of Health and Human Services (DHHS) - Contract Unit for forwarding to CDPH for final execution.
