Recommendation
That the Board of Supervisors approve the agreement with LMT & Associates Consulting, Inc. for fiscal years 2016-2017 through 2017-2018; authorize the Chair of the Board to execute three (3) originals of the agreement (Attachment 1); and direct the Clerk of the Board to return two (2) executed originals of the agreement to the Department of Health and Human Services (DHHS)-Contract Unit.
