RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve, and authorize the Public Works Director to execute, the attached change order to Contract Number 217224 for the total amount of $50,000 (4/5 Vote Required);
2. Accept the completed contracts, including all change orders approved by the Public Works Department, between Mercer-Fraser Company and Humboldt County for construction of Storm Damage Repair to Mattole Road (F3C010) Post Miles 5.00 and 13.67; and
3. Approve the final claims for payment, to be submitted by the Public Works Department to the Auditor-Controller, in the amounts of $50,000 for Post Mile 5.00 and $225 for Post Mile 13.67, which will bring the total amount paid to 100% of the contracts.

DISCUSSION:
On Mar. 7, 2017, the Governor of the State of California declared a disaster for Humboldt County due to severe winter storms, flooding, and mudslides. Mattole Road (F3C010) Post Miles 5.00 and 13.67, located in the Petrolia-Honeydew area of southern Humboldt County, experienced roadway settlement and shoulder failure. Emergency temporary repairs were performed at the time of the disaster to maintain essential traffic.
On Sept. 28, 2020, the Humboldt County Department of Public Works (Public Works) received Federal Highway Administration funding approval to proceed with the Mattole Road (F3C010) Post Miles 5.00 and 13.67 Storm Damage Repair Project. GHD, Inc., prepared plans and specifications for the project based on topographic surveys performed by Points West Surveying Company and geotechnical studies prepared by Crawford & Associates.
