RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve the grant agreement #23-10315 with CDPH in an amount up to $903,260 for the term of October 1, 2023, through September 30, 2026;

2.                     Authorize the Director of Public Health or designee to sign the attached grant agreement #23-10315;

3.                     Authorize the Director of Public Health or designee to sign any subsequent amendments and documents directly related to the grant agreement #23-10315 upon approval by County Counsel and Risk Management; and

4.                     Direct the Clerk of the Board to return one (1) copy of the certified board report to the DHHS - Contract Unit for transmittal to DHHS - Public Health.

DISCUSSION:

The County of Humboldt has been working with CDPH on nutrition education and physical activity programs since 2006. CDPH administers a portion of the United States Department of Agriculture (USDA) funded Supplemental Nutrition Assistance Program - Education (SNAP-Ed), known in California as the CalFresh Healthy Living program (CFHL).  The CalFresh Healthy Living program offers a comprehensive public health approach that enables partners to work together to prevent obesity and serve California’s individuals and families with low incomes. The target audience for CFHL is SNAP participants and other low-income individuals or families whose income is at or below 185 percent of the Federal Poverty Level. The purpose of the Grant is to provide allowable nutrition education and obesity prevention activities and interventions.  The focus of this program is upstream public health approaches, and individual and organizational health promotion to help the CFHL target audiences establish healthy eating habits, a physically active lifestyle, and for primary prevention of disease. 

 

On August 20, 2019 (item C-25), your Board approved grant agreement #19-10355 for the term of October 1, 2019, through September 30, 2022. The terms of the agreement were extended by CDPH for one year and expire on September 30, 2023.  The agreement before your Board today will replace that agreement and allow DHHS-Public Health, Healthy Communities to continue to provide physical activity and nutrition (PAN) activities and interventions to low-income Humboldt County residents.

 

The interventions and activities contained within the grant agreement include behavioral outcomes as well as physical, social, and environmental outcomes, consistent with increasing recognition of the significant role that environmental factors play in individual and group decision-making. Activities will include providing direct education using an evidence-based curriculum approved for the target population that includes objectives of physical activity promotion, healthy eating, and increased consumption of water and decreased consumption of sugar-sweetened beverages. These activities support the 2022 Community Health Improvement Plan (CHIP) priority area: Healthy Beginnings and ACEs, Goal 1: Promote a healthy start and ongoing protective factors for children and families.

Narrative Explanation of Financial Impact:

The three-year Cal Fresh Healthy Living grant is 100% federally funded and runs on a federal fiscal year (FFY).  Approval of grant agreement #23-10315 with CDPH will allow DHHS - Public Health to be reimbursed $250,906 per federal fiscal year for a total of $752,718 for the period of October 1, 2023, through September 30, 2026. The total budget maximum of $903,260 mentioned in the grant contract includes an additional 20 percent above the FFY 2024-2026 funding allocation projections, but the additional funding is not currently allocated or guaranteed to the county. This additional amount provides flexibility for budget allocations and minimizes the administrative burden in the event additional funds become available and are awarded to the County. Funding in the amount of $188,179.50 was included in the approved County budget for Fiscal Year (FY) 2023-24 and the funding amounts of $250,906.00 for FYs 2024-25 and 2025-26 and $62,726.50 for the final quarter of the grant for FY 26-27 will be included in the annual budget process in Fund 1175, budget unit 433 Nutrition & Physical Activity for FY 2024-25, 2025-26, and 2026-27.

 

STAFFING IMPACT:

The current staffing levels in the Cal Fresh Healthy Living program will not be affected by entering into this agreement as the work detailed in this contract is already being carried out by existing staff.

ALTERNATIVES TO STAFF RECOMMENDATIONS:                     

The Board could choose not to approve this grant agreement; however, such an alternative would result in a loss of both funding and services to the County.
