RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve the termination of the License with Grace Good Shepherd Church for use of premises at 1450 Hiller Road, McKinleyville; and
2. Authorize Public Works to transmit the 30-Day Termination Notice Letter (Attachment F) to Grace Good Shepherd Church.

DISCUSSION:
On April 7, 2009, your Board approved a License between Grace Good Shepherd Church and the County of Humboldt for use of a portion of the premises at 1450 Hiller Road, McKinleyville, by the Department of Health and Human Services (DHHS) for the Women, Infants and Children (WIC) Program (Attachment A). On Jan. 26, 2010, your Board approved the First License Amendment to allow the rental rate to escalate (Attachment B). The Second License Amendment was executed on Jan. 24, 2012, to extend the License to Feb. 28, 2015, and to increase the rental rate to $250 per month (Attachment C). A Third Amendment was executed on July 24, 2012, to increase the rental rate to $290 per month (Attachment D). On Feb. 24, 2015, the Fourth License Amendment increased the rental rate to $300 month and extended the term on a month-to-month basis unless either party gives thirty (30) days written notice to terminate to the other party (Attachment E).

The DHHS WIC Program shall move to the newly complete McKinleyville Center at 1615 Heartwood Drive. Therefore, DHHS no longer has use for the premises at 1450 Hiller Road. With Board approval, staff would like to terminate the License at 1450 Hiller Road by transmitting the 30-Day Termination Notice Letter (Attachment F) to Grace Good Shepherd Church.
