RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Approve, and authorize the Chair of the Board to Execute, the attached service provider agreement between the Department of Health and Human Services - Public Health (DHHS - Public Health) and Persimmony International, Inc. for the provision of electronic case management and time study plus services for the period of July 1, 2021 through June 30, 2024;
2. Approve, and authorize the DHHS - Public Health Director, or a designee thereof, to execute any and all subsequent amendments to the agreement thereto after review and approval by County Counsel, Risk Management, and the County Administrative Office; and
3. Direct the Clerk of the Board to return one fully executed copy of the attached agreement between DHHS - Public Health and Persimmony International, Inc. to the DHHS - Contracts Unit for further processing.

DISCUSSION:
On March 8, 2016 (Item C-7) the Board approved the Application Service Provider Agreement between Persimmony International, Inc. and DHHS - Public Health. On April 16, 2019 (Item C-18), the Board approved an amendment to the agreement to extend the term thereof for an additional 2 years. The attached agreement with Persimmony International, Inc. before your Board today is for the continued use of the Persimmony software, within various divisions of DHHS - Public Health, for the provision of electronic case management and required perpetual time study services. The Persimmony software is specifically designed for case management and reporting requirements of Targeted Case Management (TCM), Medical Administrative Activities (MAA), Nurse Family Partnership (NFP), and Maternal Adolescent Child Health (MCAH) Oral Health-Smile Humboldt Dental Program. Persimmony recently implemented the Time Study Plus (TSP) module for easier completion and tracking of state required perpetual time study. This implementation is an example of how Persimmony continues to increase staff efficiency while also keeping DHHS - Public Health staff moving forward with fully electronic charting, case management and time studying.
Future projects with Persimmony scheduled within this contract term include linking the TCM and TSP time study modules with the County payroll system for automatic cross-checking of staff timecards to staff time studies. This should eliminate hours of manual cross-checking currently being done by program and fiscal staff. Another future project is the ability for DHHS - Public Health Nursing programs to utilize TSP to complete documentation required for the Federal Financial Participation time study. This would establish a consistency to all DHHS - Public Health Nursing programs’ time studies and create a central location for the fiscal team to locate needed information for invoicing.
FINANCIAL IMPACT:
The renewal of the Agreement with Persimmony International Inc. will be in effect for Fiscal Year (FY) 2021-22, FY 2022-23 and FY 2023-24. The total amount of the agreement is $565,740. The amount for FY 2021-22 is $212,305. This amount was anticipated and is included in the FY 2021-22 county budget in Fund 1175, budget unit 416- Public Health Field Nursing. It includes one-time charges for creating and implementing the customized system for MCAH Oral Health-Smile Humboldt Dental Program. The amount for FY 2022-23 is $180,130 and includes one-time charges for linking the Persimmony time study programs to the County payroll system for automatic cross-checking of time studies. The amount for FY 2023-24 is $173,305 and is our estimated baseline yearly charge. The amounts for FY 2022-23 and FY 2023-24 will be included in upcoming county budgets. Approval of this agreement will not impact the General Fund.

ALTERNATIVES TO STAFF RECOMMENDATIONS:
The Board could choose not to approve and sign the renewal agreement. This is not recommended as it would impact DHHS - Public Health’s ability to maintain electronic client records, track required documentation to meet State audit requirements and streamline multiple time study processes.
