RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Chair of the Board to execute, the attached narcotic replacement therapy services agreement with Aegis Treatment Centers, LLC regarding the provision of narcotic replacement therapy services to eligible Drug Medi-Cal Beneficiaries residing in Humboldt County for the period of Feb. 1, 2017 through June 30, 2020;

2.                     Authorize the Department of Health and Human Services - Mental Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached narcotic replacement therapy services agreement with Aegis Treatment Centers, LLC, after review and approval by County Counsel, Risk Management and the County Administrative Office; and

3.                     Direct the Clerk of the Board to return one (1) fully executed original copy of the attached narcotic replacement therapy services agreement with Aegis Treatment Centers, LLC to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

Pursuant to the Substance Use Disorder Services Agreements (State Standard Agreement Nos. 17-94072 and 17-94129) that the County of Humboldt has with the California Department of Health Care Services, the Humboldt County Department of Health and Human Services (DHHS - Mental Health) is obligated to provide, or arrange for the provision of, the covered services set forth therein, including, without limitation, narcotic replacement therapy services.  There is currently a lack of any certified narcotic treatment programs in Humboldt County.  As a result, DHHS - Mental Health has historically arranged for Drug Medi-Cal beneficiaries residing in Humboldt County that meet necessary medical criteria to receive narcotic replacement therapy, and other covered services, from out-of-county providers, including without, limitation Aegis Treatment Centers, LLC.

 

Aegis Treatment Centers, LLC manages thirty-one (31) narcotic treatment programs at various outpatient clinics across California and is currently in the process of becoming licensed to operate a narcotic treatment facility in Eureka.  Drug Medi-Cal beneficiaries residing in Humboldt County have received covered treatment services from Aegis Treatment Centers, LLC on numerous occasions.  On each occasion, Aegis Treatment Centers, LLC has demonstrated expertise in providing a full range of substance use disorder treatment services, including, without limitation, narcotic replacement therapy services.

The attached narcotic replacement therapy services agreement will allow eligible Drug Medi-Cal beneficiaries residing in Humboldt County to receive narcotic replacement therapy services from Aegis Treatment Centers, LLC until June 30, 2020.  The attached narcotic replacement therapy services agreement will also allow DHHS - Mental Health to seek reimbursement from the California Department of Health Care Services for narcotic replacement therapy services provided to eligible Drug Medi-Cal beneficiaries residing in Humboldt County by Aegis Treatment Centers, LLC since Feb. 1, 2017.

 

Accordingly, DHHS - Mental Health recommends that the Board of Supervisors approve, and authorize the Chair of the Board to execute, the attached narcotic replacement therapy services agreement with Aegis Treatment Centers, LLC regarding the provision of narcotic replacement therapy services to eligible Drug Medi-Cal beneficiaries residing in Humboldt County for the period of Feb. 1, 2017 through June 30, 2020.  It is also recommended that the Board authorize the DHHS - Mental Health Director, or a designee thereof to execute any and all subsequent amendments to the attached narcotic treatment services agreement with Aegis Treatment Centers, LLC, after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid any unnecessary delays in the provision of needed narcotic replacement therapy services.

 

FINANCIAL IMPACT:

The attached narcotic replacement therapy services agreement with Aegis Treatment Centers, LLC, has a maximum amount payable of $175,948.00 for the period of Feb. 1, 2017 to June 30, 2020.  However, in the event local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the attached narcotic replacement therapy services agreement with Aegis Treatment Centers, LLC in its entirety.

 

Expenditure appropriations related to the attached narcotic replacement therapy services agreement with Aegis Treatment Centers, LLC have been included in the approved fiscal year 2019-20 budget for DHHS - Substance Use Disorder budget unit 1180-425.  Funding for the attached narcotic replacement therapy services agreement with Aegis Treatment Centers, LLC is available through federal financial participation in the Drug Medi-Cal service delivery and state general fund and behavioral health realignment revenues received by DHHS - Mental Health.  As a result, approval of the attached narcotic replacement therapy services agreement with Aegis Treatment Centers, LLC will not impact the Humboldt County General Fund.

 

STRATEGIC FRAMEWORK:

The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations, supporting self-reliance of citizens, improving the safety and health of our communities and providing appropriate levels of services.

None

 

ALTERNATIVES TO STAFF RECOMMENDATIONS:

The Board could choose not to approve the attached narcotic replacement therapy services agreement with Aegis Treatment Centers, LLC regarding the provision of narcotic replacement therapy services to eligible Drug Medi-Cal Beneficiaries residing in Humboldt County.  However, this alternative is not recommended since it would require DHHS - Mental Health to directly provide the narcotic replacement therapy services currently being provided by Aegis Treatment Centers, LLC, which is not feasible at this time.
