RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Department of Health and Human Services - Mental Health Director, or a designee thereof, to execute, the attached memorandum of understanding with Health Management Associates, Inc. regarding Humboldt County Department of Health and Human Services - Mental Health’s participation in the California Medication Assisted Treatment Expansion Project for the period of April 1, 2019 through Sept. 30, 2020; and

2.                     Authorize the Department of Health and Human Services - Mental Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached memorandum of understanding with Health Management Associates, Inc. after review and approval by County Counsel, Risk Management and the County Administrative Office.

DISCUSSION:

In 2017, Humboldt County had the second highest per capita overdose rate among California counties.  Medication assisted treatment in correctional settings has been proven to lower mortality rates upon the release of former detainees.  The provision of medication assistance in the Humboldt County Correctional Facility will support the continuity of care, and decrease withdrawal experiences, of detainees with opioid use disorder.

 

Health Management Associates, Inc. is a sub-recipient of the State Targeted Response to the Opioid Crisis Grant (STR Opioid Grant) awarded by the United States Department of Health and Human Services - Substance Abuse and Mental Health Services Administration to the California Department of Health Care Services.  As specified in the sub-recipient agreement between Health Management Associates, Inc. and the California Department of Health Care Services, Health Management Associates, Inc. has agreed to distribute up to $25,000.00 in STR Opioid Grant funds to each California county participating in the California Medication Assisted Treatment Expansion Project, for the purpose of expanding access to medication assisted treatment of opioid addiction in correctional facilities and drug courts.

 

The attached memorandum of understanding with Health Management Associates, Inc. will allow the Humboldt County Department of Health and Human Services - Mental Health (DHHS - Mental Health) to participate in the California Medication Assisted Treatment Expansion Project Learning Collaborative, which includes four in-person training sessions and access to technical assistance, such as monthly coaching calls, webinars and podcasts, regarding the provision of medication assisted treatment services, for the period of April 1, 2019 through Sept. 30, 2020.  Upon expiration of the attached memorandum of understanding with Health Management Associates, Inc., DHHS - Mental Health will have the opportunity to apply for up to $300,000.00 in additional funding that will be used to implement jail-based substance use disorder screening, assessment and medication assisted treatment, if awarded.  It should be noted that the attached memorandum of understanding with Health Management Associates, Inc. comes to the Board after the start date thereof due to inadvertent delays that occurred during the agreement development process. 

 

Accordingly, DHHS - Mental Health recommends that the Board approve, and authorize the DHHS - Mental Health Director, or a designee thereof, to execute, the attached memorandum of understanding with Health Management Associates, Inc. regarding DHHS - Mental Health’s participation in the California Medication Assisted Treatment Expansion Project for the period of April 1, 2019 through September 30, 2020.  It is also recommended that the Board authorize the DHHS - Mental Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached memorandum of understanding with Health Management Associates, Inc., after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to ensure DHHS - Mental Health’s continued participation in the California Medication Assisted Treatment Expansion Project.

 

FINANCIAL IMPACT:

The attached memorandum of understanding with Health Management Associates, Inc. will allow DHHS - Mental Health to accept $25,000.00 that will be used to support its participation in the California Medication Assisted Treatment Expansion Project Learning Collaborative.  Revenues and expenditure appropriations related to the attached memorandum of understanding with Health Management Associates, Inc. have been included in the approved fiscal year 2019-20 budgets for DHHS - Mental Health budget units 1170-424 and 1170-427 respectively.  As a result, approval of the attached memorandum of understanding with Health Management Associates, Inc. will not impact the Humboldt County General Fund.

The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations, supporting self-reliance of citizens, improving the safety and health of our communities and providing appropriate levels of services.

ALTERNATIVES TO STAFF RECOMMENDATIONS:

The Board may choose not to approve the attached memorandum of understanding with Health Management Associates, Inc. regarding DHHS - Mental Health’s participation in the California Medication Assisted Treatment Expansion Project.  However, this alternative is not recommended since the attached memorandum of understanding with Health Management Associates, Inc. will allow DHHS - Mental Health personnel to attend in-person training sessions and access technical assistance, such as monthly coaching calls, webinars and podcasts, regarding the provision of medication assisted treatment services.
