RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Chair of the Board to execute, the attached memorandum of understanding with Restpadd, Inc. regarding payment of the costs and expenses associated with the provision of additional adult inpatient psychiatric services to eligible Medi-Cal beneficiaries residing in Humboldt County;

2.                     Authorize the Department of Health and Human Services Director, or a designee thereof, to execute any and all subsequent amendments to the attached memorandum of understanding with Restpadd, Inc. after review and approval by County Counsel, Risk Management and the County Administrative Office; and

3.                     Direct the Clerk of the Board to return one (1) fully executed original copy of the attached memorandum of understanding with Restpadd, Inc. to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Board of Supervisors has authorized the Department of Health and Human Services - Mental Health (“DHHS - Mental Health”) to operate the County of Humboldt’s Mental Health Plan.  The provision of specialty mental health services to all Medi-Cal beneficiaries residing in Humboldt County that meet necessary criteria is, and will remain, the responsibility of DHHS - Mental Health.  Therefore, since April 1998, DHHS - Mental Health has contracted with qualified specialty mental health providers, including Restpadd Inc., who have historically served Medi-Cal beneficiaries residing in Humboldt County.  DHHS - Mental Health has established a provider network with in-county as well as out-of-county providers for referrals of Medi-Cal beneficiaries residing in Humboldt County to ensure that such beneficiaries have a choice of appropriate providers.

 

Psychiatric Health Facilities offer an alternative to repeated psychiatric hospitalization.  Adults placed at these facilities are stabilized, treated and transitioned to a variety of less restrictive placements as part of each individual’s long-term permanency plan.  Restpadd Inc’s Psychiatric Health Facility, located in Redding, California, is an inpatient acute care treatment facility that provides multi-disciplinary evaluation, treatment, rapid stabilization and coordinated discharge planning services to adults from eighteen (18) to sixty-four (64) years of age who are unable to maintain and safely function at home or in the community. When an initial placement at Restpadd’s Psychiatric Health Facility occurs, the client is stabilized and transitional plans are put into place and reviewed on a regular basis in order to transition the client to community living as soon as may be clinically appropriate.  All psychiatric inpatient services provided at Restpadd Inc.’s Psychiatric Health Facility are delivered with a wellness and recovery focus.

 

On November 16, 2018, the County of Humboldt entered a professional services agreement with Restpadd Inc. regarding the provision of adult inpatient psychiatric services to eligible Medi-Cal beneficiaries residing in Humboldt County for the period of May 29, 2018 through June 30, 2019.  On June 25, 2019 (Item C-30), the Board approved the first amendment to the professional services agreement with Restpadd Inc. which increased the maximum amount payable thereunder in order to cover unexpected utilization of the adult inpatient psychiatric services provided by Restpadd Inc.  However, due to the placement of additional eligible Medi-Cal beneficiaries at Restpadd, Inc’s Psychiatric Health Facility, Restpadd Inc. was required to provide additional psychiatric inpatient services during fiscal year 2018-2019 that were not included in the increased maximum amount payable set forth in the first amendment to the above-referenced professional services agreement.  Given the nature and timing of the additional psychiatric inpatient services provided by Restpadd, Inc., it was not feasible to prepare and execute a second amendment to the above-referenced professional services Agreement prior to the expiration thereof.

 

The purpose of the attached memorandum of understanding is to document Restpadd, Inc.’s provision of additional psychiatric inpatient services during fiscal year 2018-2019, and to provide for payment of such services.  Accordingly, DHHS - Mental Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached memorandum of understanding with Restpadd Inc. regarding payment of the costs and expenses associated with the provision of additional adult psychiatric inpatient services to eligible Medi-Cal beneficiaries residing in Humboldt County.  It is also recommended that the Board authorize the DHHS - Mental Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached memorandum of understanding with Restpadd Inc., after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid any unnecessary delays in the payment of the costs and expenses associated with the provision of additional inpatient psychiatric services.

 

FINANCIAL IMPACT:

The attached memorandum of understanding with Restpadd Inc. has a maximum amount payable of Five Thousand Four Hundred Dollars ($5,400.00).  Expenditure appropriations related to the attached memorandum of understanding with Restpadd, Inc. have been included in the approved fiscal year 2019-2020 budget for DHHS - Mental Health Administration budget unit 1170-424.  Funding for the attached memorandum of understanding with Restpadd Inc. is made available through federal Medi-Cal financial participation.  As a result, approval of the attached memorandum of understanding with Restpadd Inc. will not impact the Humboldt County General Fund.

 

STRATEGIC FRAMEWORK:

The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations, creating opportunities for improved safety and health and providing appropriate levels of services.

ALTERNATIVES TO STAFF RECOMMENDATIONS:

The Board could choose to not approve the attached memorandum of understanding with Restpadd, Inc. regarding payment of the costs associated with the provision of additional adult inpatient psychiatric services to eligible Medi-Cal beneficiaries residing in Humboldt County.  However, this alternative is not recommended since it would not allow DHHS - Mental Health to pay Restpadd, Inc. for the additional adult inpatient psychiatric services provided during fiscal year 2018-2019.
