RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Chair of the Board to execute, the attached professional services agreement with California Psychiatric Transitions, Inc. regarding the provision of long-term residential treatment services to adults with severe mental illnesses for the period of July 1, 2019 through June 30, 2022;

2.                     Authorize the Department of Health and Human Services - Mental Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached professional services agreement with California Psychiatric Transitions, Inc. after review and approval by County Counsel, Risk Management and the County Administrative Office; and

3.                     Direct the Clerk of the Board to return one fully executed original copy of the attached professional services agreement with California Psychiatric Transitions, Inc. to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Humboldt County Department of Health and Human Services - Mental Health (DHHS - Mental Health), in coordination with the Humboldt County Public Guardian’s Office, utilizes a variety of placement options to provide long-term mental health treatment services to mentally ill clients who require assistance in a locked and secure setting.  It is important for the placement team to have a variety of treatment options because vacancies and client needs fluctuate.

 

California Psychiatric Transitions, Inc. operates a Mental Health Rehabilitation Center (MHRC) located in Delhi, California.  California Psychiatric Transitions’ MHRC provides long-term mental health treatment services to individuals who require twenty-four hour monitoring in a locked and secure residential setting due to the severity of their mental illnesses.

 

DHHS - Mental Health clients placed at California Psychiatric Transitions’ MHRC are monitored by the county’s multi-disciplinary Strategic Assistance for Adult Recovery Interventions (SAFARI) committee.  The SAFARI committee authorizes services, determines the appropriate level of care and monitors clients’ needs in the community upon discharge.  When an initial placement at California Psychiatric Transitions’ MHRC occurs, transitional plans for a less restrictive placement are put into place, and reviewed weekly, in order to transition the client to community living as soon as may be appropriate.  All mental health treatment services provided at California Psychiatric Transitions’ MHRC are delivered with a wellness focus which includes, without limitation, concepts of recovery and resilience.

 

On June 7, 2016 (Item C-8), the Humboldt County Board of Supervisors approved the previous professional services agreement with California Psychiatric Transitions, Inc. regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses for the period of July 1, 2016 through June 30, 2019.  The Board subsequently approved the first amendment to the previous professional services agreement with California Psychiatric Transitions, Inc., which increased the maximum amount payable thereunder, on May 8, 2018 (Item C-15). 

 

The attached professional services agreement will allow eligible DHHS - Mental Health clients to receive long-term residential mental health treatment services at California Psychiatric Transitions’ MHRC for the period of July 1, 2019 through June 30, 2022.  It should be noted that the attached professional services agreement with California Psychiatric Transitions, Inc. comes to the Board after the start date thereof due to inadvertent delays that occurred during the contract negotiation process.

 

Accordingly, DHHS - Mental Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached professional services agreement with California Psychiatric Transitions, Inc. regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses for the period of July 1, 2019 through June 30, 2022.  It is also recommended that the Board authorize the DHHS - Mental Health Director, or a designee thereof, to execute any and all future amendments to the attached professional services agreement with California Psychiatric Transitions, Inc., after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid delays in the provision of needed residential mental health treatment services.

 

FINANCIAL IMPACT:

The attached professional services agreement with California Psychiatric Transitions, Inc. has a maximum amount payable of $975,000 for the period of July 1, 2019 through June 30, 2022.  In no event shall the maximum amount paid under the attached professional services agreement with California Psychiatric Transitions, Inc. exceed $325,000 per fiscal year for fiscal years 2019-2020 through 2021-2022. However, if local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the attached professional services agreement with California Psychiatric Transitions, Inc. in its entirety.

 

Expenditure appropriations related to the attached professional services agreement with California Psychiatric Transitions, Inc. have been included in the proposed fiscal year 2019-2020 budget for DHHS - Mental Health Adult Services budget unit 1170-496. Funding for the attached professional services agreement with California Psychiatric Transitions, Inc. is available through state realignment and Mental Health Services Act revenues received by DHHS - Mental Health.  As a result, approval of the attached professional services agreement with California Psychiatric Transitions, Inc. will not impact the Humboldt County General Fund.

 

STRATEGIC FRAMEWORK:

The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations, supporting self-reliance of citizens and improving the safety and health of our communities.

ALTERNATIVES TO STAFF RECOMMENDATIONS:                     

The Board could choose not to approve the attached professional services agreement with California Psychiatric Transitions, Inc. regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses. However, this alternative is not recommended since it would require DHHS - Mental Health to place clients at inpatient facilities which will be more costly and may not provide the appropriate level of treatment for such clients.
