Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Chair of the Board to execute, the attached professional services agreement with Restpadd Health Corporation regarding the provision of adult and youth psychiatric inpatient services to eligible Medi-Cal beneficiaries residing in Humboldt County for the period of July 1, 2019 through June 30, 2021;

2.                     Authorize the Department of Health and Human Services - Mental Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached professional services agreement with Restpadd Health Corporation after review and approval by County Counsel, Risk Management and the County Administrative Office; and

3.                     Direct the Clerk of the Board to return two fully executed original copies of the attached professional services agreement to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Board of Supervisors has authorized the Department of Health and Human Services - Mental Health (DHHS - Mental Health) to operate the County of Humboldt’s Mental Health Plan.  The provision of specialty mental health treatment services to all Medi-Cal beneficiaries residing in Humboldt County that meet necessary criteria is, and will remain, the responsibility of DHHS - Mental Health.  Therefore, since April 1998, DHHS - Mental Health has contracted with qualified specialty mental health providers, including Restpadd Health Corporation, who have historically served Medi-Cal beneficiaries residing in Humboldt County.  DHHS - Mental Health has established a provider network with in-county as well as out-of-county providers for referrals of Medi-Cal beneficiaries residing in Humboldt County to ensure that such beneficiaries have a choice of appropriate providers.

 

Psychiatric health facilities offer an alternative to repeated psychiatric hospitalization.  Individuals placed at these facilities are stabilized, treated and transitioned to a variety of less restrictive placements including, without limitation, family, foster families or lower level group homes as part of each individual’s long-term permanency plan. 

 

Restpadd Health Corporation’s psychiatric health facility, located in Red Bluff, California, is an inpatient acute care treatment facility that provides multi-disciplinary evaluation, treatment, rapid stabilization and coordinated discharge planning services to both adults, between 18 and 64 years of age, and adolescents, between 13 to 18 years of age, who are unable to maintain and safely function at home or in the community.  The two-unit design of Restpadd Health Corporation’s facility prevents mingling between adults and adolescents.  When an initial placement at Restpadd’s psychiatric health facility occurs, the client is stabilized and transitional plans are put into place, and reviewed on a regular basis, in order to transition the client to community living as soon as may be clinically appropriate.  All psychiatric inpatient services provided at Restpadd Health Corporation’s psychiatric health facility are delivered with a wellness focus, which includes, without limitation, concepts of recovery and resilience.

 

DHHS - Mental Health has previously contracted with Restpadd Health Corporation and its parent company, Restpadd Inc., on numerous occasions.  On each occasion, Restpadd Health Corporation and Restpadd Inc. demonstrated expertise in providing a full range of inpatient psychiatric and rehabilitative services, including, without limitation, 24-hour nursing care, dietary services, medical, psychiatric, psychological and social assessments, crisis intervention, administration and supervision of psychotropic medications, individual and group therapy, art, recreational and vocational therapy, clinical laboratory services, social services and discharge planning services.  Restpadd Health Corporation is an out-of-county organizational provider that is only utilized when necessary to serve beneficiaries with the highest level of needs. 

 

The attached professional services agreement will allow eligible Medi-Cal beneficiaries residing in Humboldt County to receive psychiatric inpatient services at Restpadd Health Corporation’s psychiatric health facility for the period of July 1, 2019 through June 30, 2021.  It should be noted that the attached professional services agreement with Restpadd Health Corporation comes to the Board after the start date thereof due to inadvertent delays that occurred during the contract negotiation process.  In order to prevent a potential break in the psychiatric inpatient services currently being provided by Restpadd Health Corporation, a Letter of Intent was executed on June 27, 2019.  

 

Accordingly, DHHS - Mental Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached professional services agreement with Restpadd Health Corporation regarding the provision of adult and youth psychiatric inpatient services to eligible Medi-Cal beneficiaries residing in Humboldt County for the period of July 1, 2019 through June 30, 2021.  It is also recommended that the Board authorize the DHHS - Mental Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached professional services agreement with Restpadd Health Corporation, after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid any unnecessary delays in the provision of required psychiatric inpatient services.

 

FINANCIAL IMPACT:

The attached professional services agreement with Restpadd Health Corporation has a maximum amount payable of $150,000 for the period of July 1, 2019 through June 30, 2021.  In no event shall the maximum amount paid under the professional services agreement with Restpadd Health Corporation exceed $75,000 per fiscal year for fiscal years 2019-2020 and 2020-2021.  However, if local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the attached professional services agreement with Restpadd Health Corporation in its entirety.

 

Expenditure appropriations related to the attached professional services agreement with Restpadd Health Corporation have been included in the proposed fiscal year 2019-2020 budget for DHHS - Mental Health Administration budget unit 1170-424.  Funding for the attached professional services agreement with Restpadd Health Corporation is made available through federal Medi-Cal financial participation and state realignment for mental health managed care and early periodic screening, diagnosis and treatment.  As a result, approval of the attached professional services agreement with Restpadd Health Corporation will not impact the Humboldt County General Fund.

 

STRATEGIC FRAMEWORK:

The recommended actions support the Board of Supervisors’ Strategic Framework by ensuring opportunities for improved safety and health, protecting vulnerable populations and providing appropriate levels of services.

ALTERNATIVES TO STAFF RECOMMENDATIONS:                     

The Board could choose not to approve the attached professional services agreement with Restpadd Health Corporation regarding the provision of adult and youth psychiatric inpatient services to eligible Medi-Cal beneficiaries residing in Humboldt County.  However, this alternative is not recommended since it would require DHHS - Mental Health to directly provide the adult and youth psychiatric inpatient services currently being provided by Restpadd Health Corporation, which is not feasible at this time.
