RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Chair of the Board to execute, the attached professional services agreement with 7th Avenue Center regarding the provision of long-term residential treatment services to adults with severe mental illnesses for the period of July 1, 2019 through June 30, 2022;

2.                     Authorize the Department of Health and Human Services - Mental Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached professional services agreement with 7th Avenue Center, after review and approval by County Counsel, Risk Management and the County Administrative Office; and

3.                     Direct the Clerk of the Board to return a fully executed original copy of the attached professional services agreement with 7th Avenue Center to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Humboldt County Department of Health and Human Services - Mental Health (DHHS -Mental Health), in coordination with the Humboldt County Public Guardian’s Office, utilizes a variety of placement options to provide long-term mental health treatment services to mentally ill clients who require assistance in a locked and secure setting.  It is important for the placement team to have a variety of treatment options because vacancies and client needs fluctuate. 

 

7th Avenue Center operates a Mental Health Rehabilitation Center (MHRC) located in Santa Cruz, California.  7th Avenue Center’s MHRC provides long-term mental health treatment services to individuals who require 24-hour monitoring in a locked and secure residential setting due to the severity of their mental illnesses.

 

DHHS - Mental Health clients placed at 7th Avenue Center’s MHRC are monitored by the county’s multi-disciplinary Strategic Assistance for Adult Recovery Interventions (SAFARI) committee.  The SAFARI committee authorizes services, determines the appropriate level of care, and monitors clients’ needs in the community upon discharge.  When an initial placement at 7th Avenue Center’s MHRC occurs, transitional plans for a less restrictive placement are put into place, and reviewed weekly, in order to transition the client to community living as soon as may be appropriate.  All mental health treatment services provided at 7th Avenue Center’s MHRC are delivered with a wellness focus which includes, without limitation, concepts of recovery and resilience.

 

On May 24, 2016 (item C-15), the Humboldt County Board of Supervisors approved the current professional services agreement with 7th Avenue Center regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses for the period of July 1, 2016 through June 30, 2019.  The attached professional services agreement will allow eligible DHHS - Mental Health clients to receive long-term residential mental health treatment services at 7th Avenue Center’s MHRC for the period of July 1, 2019 through June 30, 2022. It should be noted that the attached professional services agreement comes to the Board after the start date thereof due to extended negotiations between 7th Avenue Center and DHHS - Mental Health. 

 

Accordingly, DHHS - Mental Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached professional services agreement with 7th Avenue Center regarding the provision of long-term residential mental health treatment services for the period of July 1, 2019 through June 30, 2022.  It is also recommended that the Board authorize the DHHS - Mental Health Director, or a designee thereof, to execute any and all future amendments to the attached professional services agreement with 7th Avenue Center, after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid delays in the provision of needed residential mental health treatment services.

 

FINANCIAL IMPACT:

The attached professional services agreement with 7th Avenue Center has a maximum amount payable of $550,000 for the period of July 1, 2019 through June 30, 2022.  In no event shall the maximum amount paid under the attached professional services agreement with 7th Avenue Center exceed $183,334 per fiscal year for fiscal years 2019-20 through 2021-22However, if local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the attached professional services agreement with 7th Avenue Center in its entirety.

 

Expenditure appropriations related to the attached professional services agreement with 7th Avenue Center have been included in the proposed fiscal year 2019-20 budget for DHHS - Mental Health Adult Services budget unit 1170-496.  Funding for the attached professional services agreement with 7th Avenue Center is available through state realignment revenues received by DHHS - Mental Health.  As a result, approval of the attached professional services agreement with 7th Avenue Center will not impact the Humboldt County General Fund.

ALTERNATIVES TO STAFF RECOMMENDATIONS:                     

The Board could choose not to approve the attached professional services agreement with 7th Avenue Center regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses.  However, this alternative is not recommended since it would require DHHS - Mental Health to place clients at inpatient facilities which will be more costly and may not provide the appropriate level of treatment for such clients.
