RECOMMENDATION(S):

Recommendation

That the Board of Supervisors:

1.                     Approve, and authorize the Chair of the Board to execute, the attached professional services agreement with Mental Health Management 1, Inc., doing business as Canyon Manor, regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses for the period of July 1, 2019 through June 30, 2022;

2.                     Authorize the Department of Health and Human Services - Mental Health Director, or a designee thereof, to execute any and all subsequent amendments to the attached professional services agreement with Mental Health Management 1, Inc., doing business as Canyon Manor, after review and approval by County Counsel, Risk Management and the County Administrative Office; and

3.                     Direct the Clerk of the Board to return one fully executed original copy of the attached professional services agreement with Mental Health Management 1, Inc., doing business as Canyon Manor, to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Humboldt County Department of Health and Human Services - Mental Health (DHHS -Mental Health), in coordination with the Humboldt County Public Guardian’s Office, utilizes a variety of placement options to provide long-term mental health treatment services to mentally ill clients who require assistance in a locked and secure setting. It is important for the placement team to have a variety of treatment options because vacancies and client needs fluctuate. 

 

Mental Health Management 1, Inc., doing business as Canyon Manor, (Canyon Manor) operates a Mental Health Rehabilitation Center (MHRC) located in Novato, California. Canyon Manor’s MHRC provides mental health treatment services to individuals who require 24-hour monitoring in a locked and secure residential setting due to the severity of their mental illnesses. 

 

DHHS - Mental Health clients placed at Canyon Manor’s MHRC are monitored by the county’s multi-disciplinary Strategic Assistance for Adult Recovery Interventions (SAFARI) committee.  The SAFARI committee authorizes services, determines the appropriate level of care, and monitors clients’ needs in the community upon discharge. When an initial placement at Canyon Manor’s MHRC occurs, transitional plans for a less restrictive placement are put into place, and reviewed weekly, in order to transition the client to community living as soon as may be appropriate. All mental health treatment services provided at Canyon Manor’s MHRC are delivered with a wellness focus that includes concepts of recovery and resilience.

 

On May 24, 2016 (item C-13), the Humboldt County Board of Supervisors approved the current professional services agreement with Canyon Manor regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses for the period of July 1, 2016 through June 30, 2019. The Board subsequently approved the first and second amendments to the current professional services agreement with Canyon Manor, which increased the maximum amount payable thereunder and adjusted the rates of compensation set forth therein, on June 20, 2017 (item C-7) and July 31, 2018 (item C-24). 

 

The attached professional services agreement will allow eligible DHHS - Mental Health clients to receive long-term residential mental health treatment services at Canyon Manor’s MHRC for the period of July 1, 2019 through June 30, 2022. It should be noted that the attached professional services agreement with Canyon Manor comes to the Board after the start date thereof due to inadvertent delays that occurred during contract negotiations.

 

Accordingly, DHHS - Mental Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached professional services agreement with Canyon Manor regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses for the period of July 1, 2019 through June 30, 2022. It is also recommended that the Board authorize the DHHS - Mental Health Director, or a designee thereof, to execute any and all future amendments to the attached professional services agreement with Canyon Manor, after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid delays in the provision of needed residential mental health treatment services.

 

FINANCIAL IMPACT:

The attached professional services agreement with Canyon Manor has maximum amount payable of $550,000 for the period of July 1, 2019 through June 30, 2022.  In no event shall the maximum amount paid under the attached professional services agreement with Canyon Manor exceed $183,334 per fiscal year for fiscal years 2019-20 through 2021-22. However, if local, state or federal funding is reduced or eliminated, the County of Humboldt may reduce the above-referenced maximum amount payable or terminate the attached professional services agreement with Canyon Manor in its entirety.

 

Expenditure appropriations related to the attached professional services agreement with Canyon Manor have been included in the proposed fiscal year 2019-20 budget for DHHS - Mental Health Adult Services budget unit 1170-496. Funding for the attached professional services agreement with Canyon Manor is available through state realignment revenues received by DHHS - Mental Health. As a result, approval of the attached professional services agreement with Canyon Manor will not impact the Humboldt County General Fund.

 

STRATEGIC FRAMEWORK:

The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations, supporting self-reliance of citizens and improving the safety and health of our communities.

ALTERNATIVES TO STAFF RECOMMENDATIONS:                     

The Board could choose not to approve the attached professional services agreement with Canyon Manor regarding the provision of long-term residential mental health treatment services to adults with severe mental illnesses.  However, this alternative is not recommended since it would require DHHS - Mental Health to place clients at inpatient facilities which will be more costly and may not provide the appropriate level of treatment for such clients.
