RECOMMENDATION(S):

Recommendation

That the Board of Supervisors approve, and authorize the Chair of the Board to execute, the attached fourth amendment to the professional services agreement with Crestwood Behavioral Health, Inc. regarding the provision of long-term residential mental health treatment services to adults with mental illnesses for the period of July 1, 2016 through June 30, 2019; and direct the Clerk of the Board to return two (2) fully executed original copies of the attached fourth amendment to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Humboldt County Department of Health and Human Services - Mental Health (“DHHS - Mental Health”), in conjunction with the Humboldt County Public Guardian’s Office, utilizes a variety of placement options to provide long-term mental health treatment services to mentally ill patients who require assistance in a secure setting.  It is important for the placement team to have access to a variety of treatment options because vacancies and client needs fluctuate.

 

Crestwood Behavioral Health, Inc. (“Crestwood”) operates a variety of facilities throughout California designed to serve diverse client needs, including, without limitation, an adult residential treatment facility and a secure mental health rehabilitation center, both of which are located in the City of Eureka. 

 

Crestwood’s adult residential treatment facility, known as the Pathways program, serves as a sub-acute psychiatric residential facility, offering rehabilitative mental health services for severely and persistently mentally ill persons.  All mental health services provided by Crestwood’s Pathways program focus on client preparation to transition to independent living within their communities.

 

Crestwood’s mental health rehabilitation center (“MHRC”) serves clients who require twenty-four (24) hour monitoring in a locked and secure environment due to the severity of their mental illness.  DHHS - Mental Health clients placed at Crestwood’s MHRC are monitored by the county’s multi-disciplinary Strategic Assistance for Adult Recovery Interventions (“SAFARI”) committee.  The SAFARI committee authorizes services, determines the appropriate level of care and monitors clients’ needs in the community upon discharge.  When an initial placement at Crestwood’s MHRC occurs, transitional plans for a less restrictive placement are put into place, and reviewed monthly, in order to transition the client to community living as soon as may be appropriate.  All mental health services provided at Crestwood’s MHRC are delivered with a wellness focus which includes, without limitation, concepts of recovery and resilience.

 

On June 28, 2016 (item C-18), the Board approved the current professional services agreement with Crestwood for the period of July 1, 2016 through June 30, 2019.  The Board subsequently approved the first, second and third amendments to the professional services agreement, which increased the maximum amount payable thereunder and adjusted the rates of compensation set forth therein, on June 20, 2017 (item C-8), August 15, 2017 (item C-13) and June 5, 2018 (item C-16).  The attached fourth amendment to the professional services agreement will adjust the rates charged by Crestwood facilities throughout the state, as of July 1, 2018.

 

The attached fourth amendment to the professional services agreement with Crestwood comes to the Board after the effective date thereof due to inadvertent delays that occurred during the negotiation process.

 

Accordingly, DHHS - Mental Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached fourth amendment to the professional services agreement with Crestwood regarding the provision of long-term residential mental health treatment services.

 

FINANCIAL IMPACT:

The professional services agreement with Crestwood has a maximum amount payable of Six Million Seven Hundred Sixty Thousand Dollars ($6,760,000.00) for the period of July 1, 2016 through June 30, 2019.  In no event shall the maximum amount paid under the professional services agreement with Crestwood exceed One Million Seven Hundred Sixty Thousand Dollars ($1,760,000.00) for fiscal year 2016-2017, and Two Million Five Hundred Thousand Dollars ($2,500,000.00) per fiscal year for fiscal years 2017-2018 and 2018-2019.  The attached fourth amendment will not increase the maximum amount payable for services rendered, and costs and expenses incurred, pursuant to the terms and conditions of the professional services agreement with Crestwood.

 

Expenditure appropriations related to the attached fourth amendment to the professional services agreement with Crestwood have been included in the approved fiscal year 2018-2019 budget for DHHS - Mental Health Adult Services budget unit 1170-496.  As a result, approval of the attached amendment will not impact the Humboldt County General Fund.

 

The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations, supporting self-reliance of citizens and improving the safety and health of our communities.

 

OTHER AGENCY INVOLVEMENT:

None.

 

ALTERNATIVES TO STAFF RECOMMENDATIONS:

The Board could choose not to approve the attached fourth amendment to the professional services agreement with Crestwood regarding the provision of long-term residential mental health treatment services to adults with mental illnesses.  However, this alternative is not recommended since it would require DHHS - Mental Health to place clients at inpatient facilities which will be more costly and may not provide the appropriate level of treatment for such clients.
