RECOMMENDATION(S):

Recommendation

That the Board of Supervisors approve, and authorize the Chair of the Board to execute, the attached first amendment to the professional services agreement with Helios Healthcare, LLC, doing business as Idylwood Care Center, regarding the provision of long-term residential skilled nursing and mental health treatment services to adults with mental illnesses for the period of July 1, 2016 through June 30, 2019; and direct the Clerk of the Board to return two (2) fully executed original copies of the attached first amendment to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Humboldt County Department of Health and Human Services - Mental Health (“DHHS - Mental Health”), in coordination with the Humboldt County Public Guardian’s Office, utilizes a variety of placement options to provide long-term care to chronically mentally ill clients who require assistance in locked and unlocked facilities.  It is important for the placement team to have a variety of treatment options because vacancies and client needs fluctuate. 

 

Helios Healthcare, LLC, doing business as Idylwood Care Center, operates the Courtyard at Idylwood, a locked skilled nursing facility, and the Gardens at ldylwood, an unlocked skilled nursing facility, located in Sunnyvale, California.  Helios Healthcare, LLC’s skilled nursing facilities serve clients who require twenty-four (24) hour monitoring in a residential setting due to the severity of their mental illness.

 

Clients placed at Helios Healthcare, LLC’s skilled nursing facilities are monitored by the county’s multi-disciplinary Strategic Assistance for Adult Recovery Interventions (“SAFARI”) committee.  The SAFARI committee authorizes services, determines the appropriate level of care and monitors client needs in the community upon discharge.  When an initial placement at either of Helios Healthcare, LLC’s skilled nursing facilities occurs, transitional plans for less restrictive placement are put into place, and reviewed monthly, in order to transition the client to community living as soon as may be appropriate.  All skilled nursing and mental health services provided at Helios Healthcare, LLC’s skilled nursing facilities are delivered with a wellness focus, which includes, without limitation, concepts of recovery and  resilience.

 

On June 28, 2016 (item C-19), the Board approved the current professional services agreement with Helios Healthcare, LLC for the period of July 1, 2016 through June 30, 2019.  Approval of the attached first amendment to the professional services agreement with Helios Healthcare, LLC will adjust the daily rates of compensation set forth therein from One Hundred Twenty-Four Dollars, One Hundred Fifty Dollars and Two Hundred Seventy Dollars to One Hundred Twenty-Eight, One Hundred Fifty-Five and Two Hundred Seventy-Eight Dollars per day, as of July 1, 2018.

 

The attached first amendment to the professional services agreement with Helios Healthcare, LLC comes to the Board after the effective date thereof due to inadvertent delays that occurred during the negotiation process.

 

Accordingly, DHHS - Mental Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached first amendment to the professional services agreement with Helios Healthcare, LLC regarding the provision of long-term residential skilled nursing and mental health treatment services.

 

FINANCIAL IMPACT:

The professional services agreement with Helios Healthcare, LLC has a maximum amount payable of Three Hundred Eighty-Seven Thousand Six Hundred Thirty Dollars ($387,630.00) for the period of July 1, 2016 through June 30, 2019.  In no event shall the maximum amount paid under the professional services agreement with Helios Healthcare, LLC exceed One Hundred Twenty-Nine Thousand Two Hundred Ten Dollars ($129,210.00) per fiscal year for fiscal years 2016-2017, 2017-2018 and 2018-2019.  The attached first amendment will not increase the maximum amount payable for services rendered, and costs and expenses incurred, pursuant to the terms and conditions of the professional services agreement with Helios Healthcare, LLC.

 

Expenditure appropriations related to the attached first amendment to the professional services agreement with Helios Healthcare, LLC have been included in the approved fiscal year 2018-2019 DHHS - Mental Health Adult Services budget unit 1170-496.  As a result, approval of the attached amendment will not impact the Humboldt County General Fund.

 

The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations, supporting self-reliance of citizens and improving the safety and health of our communities.

 

OTHER AGENCY INVOLVEMENT:

None.

 

ALTERNATIVES TO STAFF RECOMMENDATIONS:

The Board could choose not to approve the attached first amendment to the professional services agreement with Helios Healthcare, LLC regarding the provision of long-term residential skilled nursing and mental health treatment services to adults with mental illnesses.  However, this alternative is not recommended since it would require DHHS - Mental Health to place clients at inpatient facilities which will be more costly and may not provide the appropriate level of treatment for such clients.
