RECOMMENDATION(S):

Recommendation

That the Board of Supervisors approve, and authorize the Department of Health and Human Services - Mental Health Director, or a designee thereof, to execute, the attached first amendment to the professional services agreement with Trustaff Travel Nurses, LLC regarding the provision of supplemental registered nursing personnel for the period of June 29, 2018 through June 30, 2021; authorize the Department of Health and Human Services - Mental Health Director, or a designee thereof, to execute any and all subsequent amendments to the professional services agreement with Trustaff Travel Nurses, LLC regarding the provision of supplemental registered nursing personnel after review and approval by County Counsel, Risk Management and the County Administrative Office; and Direct the Clerk of the Board to return two (2) fully executed original copies of the attached first amendment to the Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Humboldt County Department of Health and Human Services - Mental Health (“DHHS - Mental Health”) is responsible for providing mental health treatment services to all eligible individuals being served at DHHS - Mental Health’s Psychiatric Health Facility, Crisis Stabilization Unit, Medication Support Clinics and other county facilities, including, without limitation, the Humboldt County Juvenile Hall, the Humboldt County Correctional Facility and the North Coast Regional Facility.  In order to meet such obligations, DHHS - Mental Health has contracted with qualified professional staffing firms, including, Trustaff Travel Nurses, LLC, who have historically provided supplemental registered nursing personnel due to the difficulties associated with recruiting and retaining qualified psychiatric registered nurses in Humboldt County.

 

On April 7, 2015 (item C-13), the Board authorized the DHHS - Mental Health Director to execute professional services agreements regarding the provision of supplemental registered nursing personnel needed to fill various positions at DHHS - Mental Health facilities.  Pursuant to such authority, the DHHS - Mental Health Director has entered into professional services agreements with six (6) professional staffing firms, five (5) of which currently provide supplemental registered nursing personnel for DHHS - Mental Health’s Psychiatric Health Facility and Crisis Stabilization Unit.

 

On June 29, 2018, the DHHS - Mental Health Director executed the current professional services agreement with Trustaff Travel Nurses, LLC for the period of June 29, 2018 through June 30, 2018.  The attached first amendment to the professional services agreement with Trustaff Travel Nurses, LLC will modify the types of positions that may filled thereunder and adjust the rates of compensation set forth therein, as of July 1, 2018.  It should be noted that the attached first amendment to the professional services agreement with Trustaff Travel Nurses, LLC comes to the Board after the effective date thereof due to inadvertent delays that occurred during the negotiation process.

 

Accordingly, DHHS - Mental Health recommends the Board approve, and authorize the DHHS - Mental Health Director, or a designee thereof, to execute, the attached first amendment to the professional services agreement with Trustaff Travel Nurses, LLC regarding the provision of supplemental registered nursing personnel to fill various positions at DHHS - Mental Health facilities.  DHHS - Mental Health also recommends that the Board authorize the DHHS - Mental Health Director, or a designee thereof, to execute any and all subsequent amendments to the professional services agreement with Trustaff Travel Nurses, LLC, after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid potential delays in the provision of needed supplemental registered nursing personnel.

 

FINANCIAL IMPACT:

The professional services with Trustaff Travel Nurses, LLC has a maximum amount payable of Two Million Four Hundred Thousand Dollars ($2,400,000.00) for the period of June 29, 2018 through June 30, 2021.  In no event shall the maximum amount paid under the professional services agreement with Trustaff Travel Nurses, LLC exceed Eight Hundred Thousand Dollars ($800,000.00) per fiscal year for fiscal years 2018-2019, 2019-2020 and 2020-2021.  The attached first amendment will not increase the maximum amount payable for services rendered, and costs and expenses incurred, pursuant to the terms and conditions of the professional services agreement with Trustaff Travel Nurses, LLC.

 

An appropriation transfer request will be submitted to transfer balances from salary and benefit line items to insure sufficient services and supplies expenditure appropriation in the approved DHHS - Mental Health Administration budget unit 1170-424 for fiscal year 2018-2019.  Revenue to support these expenditures is available through Medicare and Medi-Cal federal financial participation, Mental Health Services Act, Realignment and private insurance. 

 

The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations, creating opportunities for improved health and safety and providing appropriate levels of services.

 

OTHER AGENCY INVOLVEMENT:

None.

 

ALTERNATIVES TO STAFF RECOMMENDATIONS:

The Board could choose to not approve the attached first amendment to the professional services agreement with Trustaff Travel Nurses, LLC regarding the provision of supplemental registered nursing personnel to fill various positions at DHHS - Mental Health facilities.  However, this alternative is not recommended since it may delay the provision of professional psychiatric services to eligible individuals being served at DHHS - Mental Health facilities.
