RECOMMENDATION(S):

Recommendation

That the Board of Supervisors approve, and authorize the Chair of the Board to execute, the attached first amendment to the professional services agreement with Montgomery Consulting regarding the provision of technical assistance with the evaluation and implementation of mental health and substance use disorder services; authorize the Humboldt County Health and Human Services Director to execute any and all future amendments to the professional services agreement with Montgomery Consulting after review and approval by County Counsel, Risk Management; and direct the Clerk of the Board to return two (2) fully executed original copies of the attached first amendment to the Humboldt County Department of Health and Human Services - Contract Unit for further processing.

DISCUSSION:

The Humboldt County Department of Health and Human Services - Mental Health (“DHHS - Mental Health”) provides inpatient and outpatient mental health, substance use disorder and other mandated and specially funded services to individuals with serious mental illness in Humboldt County.  Provision of mental health services and funding for public mental health programs are both complex at their core and changing laws and regulations have compounded the issues in public mental health administration.

 

Dr. Mark Montgomery of Montgomery Consulting has a bachelor’s degree in public administration, a master’s degree in psychology and a doctorate degree in clinical psychology.  Having served as the Shasta County Mental Health Director and a behavioral health and substance use disorder consultant for the past ten (10) years, Dr. Montgomery has in-depth knowledge of the specific challenges of a rural northern California health and human services agency.  Dr. Montgomery’s knowledge of service delivery, systems, funding and applicable laws and regulations uniquely qualifies him to provide professional consulting services to DHHS - Mental Health. 

 

Dr. Montgomery has been providing consulting services to DHHS since August of 2017 when he was brought on to support the newly-promoted DHHS - Mental Health Director.  Dr. Montgomery worked with DHHS - Mental Health staff to conduct an assessment of the mental health and substance use disorder services currently being provided by the county which focused on inpatient and outpatient adult mental health programs, compliance and quality assurance activities, staff training and modernization of the county’s policies, procedures, protocols and guidelines pertaining to the provision of mental health and/or substance use disorder services.  Dr. Montgomery has also provided technical assistance, coaching and mentoring to the DHHS - Mental Health Director to ensure that she had the support necessary to make a successful transition to adult mental health services. 

 

The attached first amendment to the professional services agreement with Montgomery Consulting extends the term thereof to June 30, 2019, and modifies the scope of services to be provided thereunder to reflect the county’s current needs. Upon execution of the attached first amendment, Montgomery Consulting will continue to provide technical assistance to enhance the administration of DHHS - Mental Health. 

 

Accordingly, DHHS - Mental Health recommends that the Board approve, and authorize the Chair of the Board to execute, the attached first amendment to the professional services agreement with Montgomery Consulting regarding the provision of technical assistance with the evaluation and implementation of mental health and substance use disorder services.  Staff also recommends that the Board of Supervisors authorize the Humboldt County Health and Human Services Director to execute subsequent amendments to the professional services agreement with Montgomery Consulting, after review and approval by County Counsel, Risk Management and the County Administrative Office, in order to avoid any unnecessary delays in the provision of required consulting services.

 

FINANCIAL IMPACT:

The attached first amendment to the professional services agreement with Montgomery Consulting will increase the maximum amount payable thereunder to Two Hundred Eighty-Six Thousand Dollars ($286,000.00) for the period of August 22, 2017 through June 30, 2019.  However, if local, state or federal funding is reduced or eliminated, the county may reduce such maximum amount payable or terminate the professional services agreement with Montgomery Consulting in its entirety. 

 

Expenditure appropriations related to the attached first amendment to the professional services agreement with Montgomery Consulting have been included in the approved fiscal year 2018-2019 budget for DHHS - Mental Health Administration budget unit 1170-424.  Funding for the attached first amendment to the professional services agreement with Montgomery Consulting is available through Medi-Cal, Drug Medi-Cal, Mental Health Services Act and Realignment revenues received by DHHS - Mental Health.  As a result, approval of the attached first amendment to the professional services agreement with Montgomery Consulting will not impact the Humboldt County General Fund.

 

The recommended actions support the Board of Supervisors’ Strategic Framework by protecting vulnerable populations and creating opportunities for improved safety and health.

ALTERNATIVES TO STAFF RECOMMENDATIONS:

The Board could choose not to approve the attached first amendment to the professional services agreement with Montgomery Consulting regarding the provision of technical assistance with the evaluation and implementation of mental health and substance use disorder services.  This alternative is not recommended since the services that will be provided by Montgomery Consulting are necessary to ensure that DHHS - Mental Health has the support needed to continue improving the county’s current mental health and substance use disorder programs.
