Subaward Agreement with the Partners in Care Foundation for Fiscal Years 2017-2021
Recommendation
That the Board of Supervisors approve the Subaward Agreement with the Partners in Care Foundation in the amount of $23,500 for the period of August 1, 2017 to July 31, 2020; authorize the Chair of the Board to sign two (2) originals of the signature page of the agreement; authorize the Director of Public Health or designee to approve all future amendments and documents directly related to those agreements, that do not require matching funds from the county to agreement upon review and approval of County Counsel and Risk Management; and direct the Clerk of the Board to return one (1) copy of the certified board report to the Department of Health and Human Services (DHHS)-Contract Unit.
