Professional Services Agreement with Humboldt County Office of Education for Nutrition Education and Obesity Prevention Program
Recommendation
That the Board of Supervisors approve the Professional Service Agreement with the Humboldt County Office of Education (HCOE) in the amount of $55,225 in Fund 1175, Budget Unit 433, Nutrition and Physical Activity for the term of October 1, 2017 through September 30, 2018; authorize the Chair of the Board to sign three (3) originals of the agreement; and direct the Clerk of the Board to return two (2) fully executed original agreements to the Department of Health and Human Services (DHHS), Contract Unit. Authorize the Director of Public Health or designee to approve all future amendments to the grant agreement that do not require matching funds from the county upon approval by County Counsel and Risk Management.
