Recommendation
That the Board of Supervisors approve the proposed Agreement with Exemplar Human Services, LLC for fiscal year 2017-2020 to provide customized automated performance management reports for the Humboldt County Department of Health and Human Services; authorize the Chair to execute three (3) originals of the Agreement; and direct Clerk of the Board to route two (2) fully executed originals to the Department of Health and Human Services (DHHS) - Contract Unit for forwarding to the Director of DHHS - Social Services
