Approval of Agreement #12-17EVRGRN for Targeted Case Management with the California Department of Health Care Services Beginning July 1, 2017
Recommendation
That the Board of Supervisors approve and authorize Agreement #12-17EVRGRN with the California Department of Health Care Services ("DHCS") concerning Targeted Case Management ("TCM") for the period beginning July 1, 2017; direct the Chair of the Board to sign three (3) originals of Agreement #12-17EVRGRN and the HIPAA Business Associate Addendum; authorize the Public Health Director, or designee, to sign the DHCS6208 PED Medi-Cal Provider Agreement; authorize the Public Health Director, or designee, to sign any future amendments to this agreement upon County Counsel and Risk Management review and approval; and direct the Clerk of the Board to return three (3) signed copies of Agreement #12-17EVRGRN, and the HIPAA Business Associate Addendum, to the Department of Health and Human Services (DHHS) - Contracts Unit for submittal to DHCS for their signature.
