Second Amendment to the Pharmacy Services Agreement with Diamond Drugs, Incorporated Dated June 23, 2015
Recommendation
That the Board of Supervisors approve, and authorize the Chair to execute, the attached second amendment to the pharmacy services agreement with Diamond Drugs, Incorporated dated June 23, 2015; and direct the Clerk of the Board to return two (2) signed originals of the attached second amendment to the Department of Health and Human Services ("DHHS")-Contract Unit for forwarding to DHHS-Mental Health Administration.
