Third Amendment to the Hospital Services Agreement with St. Joseph Hospital of Eureka
Recommendation
That the Board of Supervisors approve and authorize the Board Chair to execute three (3) original copies of the attached third amendment to the agreement with St. Joseph Hospital of Eureka for hospital medical services ("Hospital Services Agreement"), which extends the term of such agreement to June 30,2020; and direct the Clerk of the Board to return two (2) signed original copies of the attached third amendment to the Humboldt County Department of Health and Human Services - Mental Health Administration for further processing.
