State Standard Agreement with California Department of Parks and Recreation for Water Testing by the Public Health Laboratory
Recommendation
That the Board of Supervisors approve state standard agreement C1637037 with the California Department of Parks and Recreation (CDPR) for water testing by the Public Health Laboratory for the North Coast Redwoods District for up to $25,000 to commence upon Notice to Proceed through March 31, 2019; authorize the Chair to sign three (3) originals of the agreement and one (1) contractor certification clause (CCC-307); authorize the Public Health Director to sign future amendments or budget revisions to this agreement; direct the Clerk of the Board to return three (3) signed agreements and one (1) CCC-307 to Department of Health and Human Services (DHHS)-Contract Unit to forward to DHHS-Public Health; and direct the Clerk of the Board to return one (1) copy of the Board Order to DHHS-Public Health for submission to CDPR.
