State Standard Agreement with California Department of Public Health, Office of AIDS for HIV/AIDS Surveillance
Recommendation
That the Board of Supervisors approve State Standard Agreement number 16-10770 with California Department of Public Health (CDPH), Office of AIDS for $38,313 for the period of July 1, 2016 through June 30, 2019; direct the Chair of the Board to sign four (4) originals of the cover page of the Agreement, the CCC-307 Contractor Certification, and Darfur Contracting Act; direct the Clerk of the Board to return the four (4) signed originals and Certifications to Department of Health and Human Services (DHHS) - Contract Unit for submission to Public Health; and direct the Clerk of the Board to return one (1) copy of the Board Order to DHHS-Contract Unit for submission to DHHS-Public Health.
