State Standard Agreement for the Core Sexually Transmitted Diseases (STD) Program Management Project for Fiscal Years 2016-2019
Recommendation
That the Board of Supervisors approve and authorize the Chair to sign two (2) original signature pages of State Standard Agreement Number 16-10714 with the California Department of Public Health (CDPH) - STD Control Branch in the amount of $12,067 in Fund 1175, Budget Unit 419 for the period of July 1, 2016 through June 30, 2019; authorize the Director of Public Health, or their designee to sign all subsequent amendments and documents directly related to this agreement, with County Counsel and Risk Management review and approval; and direct the Clerk of the Board to send a fully executed agenda item and the original signed documents to the Department of Health and Human Services (DHHS) Contract Unit for transmittal to DHHS - Public Health.
