Grant Agreement with the CUPA Forum Environmental Protection Trust Fund for $30,458.75 for the Purchase of an Emergency Response Vehicle (4/5 Vote Required)
Recommendation
That the Board of Supervisors approve Grant Agreement No. 2016-1 with the CUPA Forum Environmental Protection Trust Fund to accept the grant amount of $30,458.75 for the purchase of an emergency response vehicle (Attachment 1); authorize the Public Health Director, or his/her designee, to sign Grant Agreement No. 2016-1 (Attachment 1); authorize the Public Health Director, or his/her designee to sign any grant invoices, reports and any subsequent amendments directly related to Grant Agreement No. 2016-1; direct the Auditor-Controller to post the supplemental budget after fiscal year 2016-17 budget adoption in Fund 1175, Budget Unit 411, Hazardous Materials Program as outlined on Attachment 2 (4/5 vote required); authorize the purchase of one emergency response vehicle for the Certified Unified Program Agency (CUPA) of the Department of Health and Human Services (DHHS), Division of Environmental Health (DEH); authorize the donation of the emergency response vehicle to the Humboldt County Motor Pool for capitalization, maintenance and insurance purposes; and direct the Clerk of the Board to return one original, signed copy of this agenda item to the DHHS Contract Unit for forwarding to DEH.
