Agreement between Humboldt County and California Psychiatric Transitions Inc. for Fiscal Years 2016-17, 2017-18, and 2018-19
Recommendation
That the Board of Supervisors approve the Agreement between Humboldt County and California Psychiatric Transitions, Inc. for fiscal years 2016-17, 2017-18, and 2018-19; authorize the Chair to execute three (3) originals of the Agreement effective July 1 2016; authorize the Department of Health and Human Services (DHHS) Director of Mental Health or designee to sign placement documents in a form similar to those attached as schedule C to the attached Agreement; and direct the Clerk of the Board to return two (2) executed originals of the Agreement to the DHHS-Contract Unit for forwarding to DHHS-Mental Health.
