Agreement with California Department of Public Health (CDPH) for the Immunization Program for Fiscal Years 2015-2017
Recommendation
That the Board of Supervisors approve agreement #15-10421 with the California Department of Public Health (CDPH) for the Immunization Program in Fund 1175, Budget Unit 428 for fiscal year 2015-17 in the amount of $182,064; authorize the Chair of the Board to sign three (3) originals of the attached agreement and certifications; and direct the Clerk of the Board to return the signed documents and one (1) copy of the Board Motion to the Department of Health and Human Services Contract Unit for transmittal to DHHS-Public Health.
