AIDS Drug Assistance Program Funding for Fiscal Years 2015-17 and Supplemental Budget for Fiscal Year (FY) 2015-16 (4/5 Vote Required)
Recommendation
That the Board of Supervisors approve and authorize the Chair to sign four (4) original signature pages of State Standard Agreement 15-10478 with the California Department of Public Health (CDPH)-Office of AIDS in the amount of $10,252 in Fund 1175, Budget Unit 437-CARE North Coast AIDS Project (NorCAP) for the period of July 1, 2015 through June 30, 2017; authorize the Chair to sign the Darfur Contracting Act Certification, the CC-307 Contractor Certification, and the Payee Data Record (STD.204); direct the Clerk of the Board to return all signed documents to Department of Health and Human Services (DHHS) Contract Unit; and direct the Auditor-Controller to supplement Fund 1175, Budget Unit 437 - CARE NorCAP in the amount of $5,126 per Attachment A.
