Agreement with Victor Treatment Centers, Inc. (Chico, California) for the Provision of Mental Health Services to Eligible Medi-Cal Beneficiaries for Fiscal Year 2015-16
Recommendation
That the Board of Supervisors approve and authorize the Agreement with Victor Treatment Centers, Inc. for the provision of Children's Mental Health Services for Fiscal Year 2015-2016, effective July 1, 2015; approve the e-signature agreement and e-signature certification form (attached to this agreement as Exhibit C); approve the Mental Health Director or designee to be generally authorized to sign health records' electronic signature agreement forms and certifications and any other necessary documents to agree to electronic signatures; authorize the Chairperson to execute three (3) originals of the Agreement; and direct the Clerk of the Board to route two (2) fully executed originals of the Agreement to the DHHS-Contract Unit for forwarding to DHHS-Social Services Administration.
