Grant Agreement with First 5 Humboldt for the Nurse-Family Partnership Program July 1, 2015 through June 30, 2017
Recommendation
That the Board of Supervisors approve and authorize the Chair to sign three (3) originals of the Grant Agreement with First 5 Humboldt in the amount of $133,610 for Nurse-Family Partnership (NFP) for the term of July 1, 2015 through June 30, 2017; and direct the Clerk of the Board to return three (3) signed originals of the Agreement to the Public Health Branch for final execution.
