First Amendment to Agreement with Star View Adolescent Center for the Provision of Mental Health Services for the Period March 31, 2014 through September 30, 2015
Recommendation
That the Board of Supervisors authorize the Chairperson to execute three (3) originals of the First Amendment to the August 12, 2014 Agreement with Star View Adolescent Center for the provision of Mental Health Services (Attachment 1) to extend the term of the Agreement through September 30, 2015 and increase the contract maximum for fiscal year 2014-15 to $136,000.15 and fiscal year 2015-16 to $48,321.00; and direct the Clerk of the Board to route two (2) fully executed originals of the First Amendment to the Agreement to the DHHS-Contract Unit for forwarding to DHHS-Children and Family Services Administration.
