First Amendment to the Service Agreement with St. Joseph Hospital of Eureka for the Period of January 1, 2015 through June 30, 2016
Recommendation
That the Board of Supervisors approve and authorize the Chair to sign three (3) originals of the First Amendment to the Service Agreement with St. Joseph Hospital of Eureka for hospital medical services for the period of January 1, 2015 through June 30, 2016; and direct the Clerk of the Board to return two (2) signed originals of the Agreement to the Department of Health and Human Services (DHHS)-Contract Unit for forwarding to DHHS-Mental Health Administration.
