Grant Agreements for Services with Community/Family Resource Centers for CalWORKs Outreach and Support and CalFresh Outreach and Support for Fiscal Year 2015-16
Recommendation
That the Board of Supervisors approve the Grant Agreements, totaling $925,000, with the Community/Family Resource Centers participating with CalWORKs Outreach and Support and CalFresh Outreach and Support for fiscal year 2015-16; authorize the Department of Health and Human Services (DHHS) Director of Social Services to execute these Grant Agreements substantially in the form of the attached sample copy of the Grant Agreement upon proof of insurance; authorize the DHHS-Director of Social Services to amend the Grant Agreement form as necessary for fiscal year 2015-16 after review and approval by County Counsel and Risk Management; and direct the Clerk of the Board to return one (1) executed Board Agenda Item to the DHHS-Contract Unit for transmittal to DHHS-Social Services Administration.
