Professional Services Agreement with Humboldt County Office of Education for Nutrition Education and Obesity Prevention Program
Recommendation
That the Board of Supervisors approve the Professional Services Agreement between the Department of Health and Human Services (DHHS)-Public Health and Humboldt County Office of Education (HCOE) in the amount of $158,538 for the term of October 1, 2014 through September 30, 2015; authorize the Chair of the Board to sign three (3) originals of the signature page of the Agreement; authorize the Director of Public Health to approve all future amendments and documents directly related to the Agreement; and direct the Clerk of the Board to return two (2) signed originals to DHHS-Contracts Unit for forwarding to DHHS-Public Health.
