Agreement between Humboldt County and Crestwood Behavioral Health, Inc. Bridgehouse for Fiscal Years 2014-15 and 2015-16
Recommendation
That the Board of Supervisors approve the Agreement between Humboldt County and Crestwood Behavioral Health, Inc. - Bridgehouse for fiscal years 2014-15 and 2015-16; authorize the Chair to execute three (3) originals of the Agreement; and direct the Clerk of the Board to return two (2) executed originals of the Agreement to the Department of Health and Human Services (DHHS)-Contract Unit for forwarding to DHHS-Mental Health Administration.
