Agreement with Crestwood Behavioral Health Inc. Placement Facilities for Fiscal Years 2014-16
Recommendation
That the Board of Supervisors approve the Agreement between County of Humboldt and Crestwood Behavioral Health, Inc. (CBHI) Placement Facilities for fiscal years 2014-16; authorize the Chair to execute three (3) originals of the Agreement effective July 1, 2014; and direct the Clerk of the Board to return two (2) executed originals of the Agreement to the Department of Health and Human Services-Contract Unit for forwarding to DHHS-Mental Health.
