RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Adopt the attached resolution authorizing the Department of Health and Human Services Director, or a designee thereof, to enter into, execute and deliver any and all applications, agreements, amendments, certifications, attestations and other documents necessary to apply for, and accept, if awarded, up to $45,000,000 in grant funding made available through the California Department of Health Care Services' Behavioral Health Continuum Infrastructure Program for the construction of a new Psychiatric Health Facility in Humboldt County;
2. Approve, and authorize the Director of Public Works to advertise and distribute, the attached request for qualifications regarding the provision of architecture and engineering services related to the construction of a new Psychiatric Health Facility in Humboldt County;
3. Authorize the Director of Public Works to execute a consultant services agreement with the successful consultant regarding the provision of architecture and engineering services related to the construction of a new Psychiatric Health Facility in Humboldt County for an amount not to exceed $150,000, and any subsequent amendments thereto, that is substantially similar to the sample consultant services agreement contained in the attached request for qualifications, after review and approval by the Department of Health and Human Services Behavioral Health, County Counsel, Risk Management and the County Administrative Office; and
4. Direct the Clerk of the Board to return one (1) fully executed certified copy of the attached resolution to the Department of Health and Human Services - Behavioral Health for submission to the California Department of Health Care Services.
