Recommendation
That the Board of Supervisors approve the Agreement between Humboldt County and Crestwood Behavioral Health Inc., Pathways for Fiscal Years 2015-16, 2016-17 and 2017-18; authorize the Chair to execute three (3) originals of the Agreement effective December 21, 2015; and direct the Clerk of the Board to return two (2) executed originals of the Agreement to the DHHS-Contract Unit for forwarding to DHHS-Mental Health.
