Agreement between Humboldt County and 7th Avenue Mental Health Rehabilitation Center for Fiscal Years 2014-2015 and 2015-2016
Recommendation
That the Board of Supervisors approve the Agreement between Humboldt County and 7th Avenue Rehabilitation Center; authorize the Chair to execute three (3) originals of the Agreement effective July 1, 2014; and direct the Clerk of the Board to return two (2) executed originals of the Agreement to the Department of Health and Human Services (DHHS)-Contract Unit for forwarding to DHHS-Mental Health Administration.
