RECOMMENDATION(S):
Recommendation
That the Board of Supervisors:
1. Authorize the Chair of the Board to execute the attached certification statements for the Child Health and Disability Prevention, Health Care Program for Children in Foster Care and California Children's Services Programs; and
2. Direct the Clerk of the Board to return the three (3) fully executed original copies of the attached certification statements to the Department of Health and Human Services - Public Health Administration for submission to the California Department of Health Care Services.

DISCUSSION:
The Nursing Division of the Humboldt County Department of Health and Human Services - Public Health ("DHHS-PH") has been administering the Child Health and Disability Prevention ("CHDP") and California Children's Services ("CCS") programs locally since 1980 and the Health Care Program for Children in Foster Care ("HCPCFC") since 2000.
CHDP is a preventive program that delivers periodic health assessments and services to low-income children and youth in California. CHDP provides care coordination to assist families with medical appointment scheduling, transportation and access to diagnostic and treatment services. Health assessments are provided by enrolled private physicians, community clinics, managed care plans and some local school districts.
HCPCFC is a program in which DHHS-PH, DHHS-Social Services and the Humboldt County Probation Department work together to provide public health nurse expertise in meeting the medical, dental, mental and developmental needs of children and youth in foster care.
