Master Services Agreement (MSA) with Relias Learning, Limited Liability Company (LLC) and Amendment to MSA with Relias Learning, LLC
Recommendation
That the Board of Supervisors approve the Master Services Agreement and related Purchase Schedule between Department of Health and Human Services (DHHS)-Mental Health and Relias Learning, LLC for the term April 1,2016 to March 31, 2019; authorize the Chair to sign three (3) originals of the Purchase Schedule between Department of Health and Human Services (DHHS)-Mental Health and Relias Learning, LLC for the term April 1,2016 to March 31, 2019; approve and authorize the Chair to sign three (3) originals of the Contract Amendment between DHHS-Mental Health and Relias Learning, LLC for the term April 1, 2016 to March 31, 2019; authorize the Director of DHHS-Mental Health to sign subsequent amendments or documents related to the Master Services Agreement, Purchase Schedule, and Amendment on behalf of Humboldt County; and direct the Clerk of the Board to return two (2) signed originals of the Master Services Agreement and related Purchase Schedule, two (2) signed originals of the Amendment, and one (1) copy of the Board motion to the DHHS-Contract Unit for transmittal to DHHS-Mental Health.
